Any travel fee assigned is based on one-way mileage:
 under 10 miles: $25
 under 20 miles: $40
under 30 miles: $60

I do not accept Insurance as a form of payment for services, however I am happy to give you documentation to submit to your insurance company for reimbursement and or utilize a flexible spending account.
